Greetings to our 2025-2026 parents/guardians of Rio Grande City Grulla I. S. D. Student-Athletes:

In an effort to stream line the required UIL Athletic paper work, Rio Grande City Grulla ISD is offering online forms. At the beginning of each sport and school year it is mandatory that all student-athletes fill out the appropriate UIL and Rio Grande City Gulla ISD paper work before they will be allowed to practice or try out in sports or an athletic period. This website is designed to stream line the process, allowing for less shuffling of papers and more communication between our schools and our athletic department.

The following forms can be completed online:

You MUST have your student-athlete’s 6 DIGIT School ID number and Full legal name provided to the school available as it is required on each form (must be a 6 digit ID#)

Please take a minute and follow read the Parent Account Tutorial to start this process of getting your student-athlete ready to participate.

The parent/guardian will be asked to provide information such as home phone numbers, cell phone numbers, email addresses, name of the primary physician, insurance policy information and other pertinent information. Please make sure you have this with you when filling out your forms electronically. Things to remember PLEASE DO NOT LEAVE ANY BOXES BLANK! (ex. If you do not have a Business # type: N/A or none in the box. If you leave a blank the system will continue to ask for some sort of information.)

Parents are encouraged to create an account with this software in order to access documents for later use of the system

Remember that to submit online appropriately, you and your child must fill out and sign in all required spaces. Forms will not submit electronically without all required information being completed.

*To access the online forms hold your cursor over the “Forms/Rank One Portal” tab. You will get a drop down list of the forms.

*Click on the form name and fill out the information requested.

*To sign the document click inside the signature box and hold your mouse down, this will allow you to create an “Electronic Signature”. If you make a mistake and need to start over click on the refresh icon next to the signature box.

*Once you have filled out all of the information on each page, you will have the opportunity to print the document. Please note that information cannot be ‘Saved as a Draft” – all forms must be completed in one session.

*You will receive a confirmation email from Rank One once your paperwork is submitted and complete.

*You will receive a confirmation email again - once all documents have been reviewed by the RGCGISD Athletic Trainers staff.

*Note for physicals: Students are required to use the Pre-participation - Physical Examination Form provided by UIL website. ( https://www.uiltexas.org/files/athletics/PrePhysFormRvsd2.21.pdf )

*Our Athletic Trainers Office will also have copies of these available upon request. Please note that physicals do not expire for 2 years. Please check with your athletic training staff about your up to date physical expiration date.
